• Blackboard Login Information - Your login information for Blackboard. This will be used to associate your account with this course. If you do not specify any information here it will be assumed that you do not have any account information available.
• Availability for Training - If training is required; we will use this information to determine your availability for training on using Blackboard.
• Course Materials - Information on how to submit your course material to be put into this course and any course materials that you need to have rolled over from a previous course.
• Comments - Any additional information we will need to properly implement this course on Blackboard.
